Jowls and Sagging Jawline

Jowls develop when skin and deeper facial tissues descend around the lower cheeks and jaw. This can interrupt the line between the chin and neck, create heaviness beside the mouth and make the lower face appear less defined.

Jowls treatment should address the structures causing the change, not just the skin’s appearance. At COSMEX, we assess the lower face, jawline and neck together before determining whether a facelift, neck lift or a combination of both may be appropriate.

What Are Jowls?

As the lower cheeks begin to descend, the jawline can lose its smooth outline, and loose skin and tissue may gather on either side of the chin. These areas are known as jowls. They may be more noticeable on one side and can occur alongside broader laxity across the lower face or beneath the jaw.

What Causes Jowls and a Sagging Jawline?

The lower face changes as the skin produces less collagen and elastin and the tissues beneath it lose support. Facial fat, ligaments, muscle and bone structure can all influence where sagging develops and how visible it becomes.

Factors that may contribute to jowls and jawline sagging include:

  • Reduced collagen and skin elasticity
  • Descent of cheek and lower-face tissue
  • Changes in facial fat distribution
  • Reduced structural support through the face and jaw
  • Genetics and natural facial anatomy
  • Repeated sun exposure
  • Smoking
  • Significant changes in weight
  • Natural ageing

How We Assess Jowls and Jawline Sagging at COSMEX

Assessment begins with an examination of the cheeks, lower face, jawline, and neck as connected areas. We consider skin elasticity, the position of deeper tissues, the amount of laxity beside the mouth and whether the concern continues beneath the jaw into the neck.

We also review your medical history, previous facial procedures, smoking or nicotine use and what you would like to change. This helps us distinguish jowls caused primarily by lower-face tissue descent from concerns involving loose neck skin, muscle laxity or fullness beneath the chin.

Jowls and Sagging Jawline Treatment Options at COSMEX

Surgical treatment is selected according to where the sagging begins and how far it extends. A facelift addresses descended tissue through the lower face, while a neck lift may be added or performed separately when laxity continues below the jawline.

Facelift Surgery for Jowls and Lower-Face Sagging

A facelift repositions and supports deeper facial tissues that have descended through the cheeks and lower face. Depending on the technique and individual anatomy, the procedure may reduce the appearance of jowls, improve jawline definition and address folds created by tissue descent. Incision placement and the extent of surgery are determined during consultation.

Neck Lift Surgery When Sagging Extends Below the Jawline

A neck lift addresses loose skin, deeper tissue laxity and muscle banding beneath the jaw and through the neck. It may improve the transition between the lower face and neck when sagging extends beyond the jawline. A neck lift does not replace a facelift when established jowls originate in the lower face, so both procedures may be considered when both regions are affected.

What to Expect From Your Jowls Treatment

Your treatment pathway will depend on whether the concern is limited to the lower face or involves the neck as well. We explain the recommended procedure, its limitations, potential risks, permanent scars and expected recovery before you decide whether to proceed.

Your Jowls Treatment Consultation

Your consultation includes an assessment of your facial structure, skin quality and the position of the tissues across the cheeks, jawline and neck. We will discuss your medical history, previous facial procedures and the changes you would like to consider before explaining whether a facelift, neck lift or combined approach may be suitable.

Your Facelift or Neck Lift Procedure

Facelift and neck lift surgery may involve incisions around the ears, within the hairline or beneath the chin, depending on the procedure selected. The deeper tissues are repositioned or tightened as required, and excess skin may be removed without relying on excessive tension across the surface. Surgery is performed under anaesthesia in an appropriately licensed facility.

Jowls Surgery Recovery

Swelling, bruising, tightness and changes in sensation can occur during the early recovery period. Activity is increased gradually, with driving, work and exercise resumed according to the procedure performed and how healing progresses. Follow-up appointments allow us to monitor the incisions, swelling and developing facial contour.

Jowls Treatment Results

Changes in the lower face and jawline become easier to assess as swelling subsides and the tissues settle. Scars continue to mature over several months, while the final degree of improvement depends on facial anatomy, skin quality, the extent of tissue descent and individual healing. Surgery cannot stop future ageing or guarantee a particular facial shape.

Why Choose COSMEX for Your Jowls Treatment?

At COSMEX, jowls treatment begins with a detailed assessment of the lower face and neck. Our surgeons consider the position of the facial tissues, skin quality and the extent of sagging before recommending an approach suited to your anatomy.

Specialist Plastic Surgeons

Our surgeons hold Fellowship of the Royal Australasian College of Surgeons in Plastic and Reconstructive Surgery. They have the specialist training required to assess facial ageing and perform facelift and neck lift procedures with an understanding of the skin, deeper tissues and underlying facial structure.

Lower-Face and Neck Assessment

Jowls may develop alongside loose neck skin, muscle laxity or reduced definition beneath the jaw. We assess the lower face and neck together to determine whether treatment should involve a facelift, neck lift or both areas during the same procedure.

Accredited Surgical Facilities in Sydney

Facelift and neck lift procedures are performed in licensed, accredited hospitals in Sydney. These facilities meet Australian standards for anaesthesia, theatre safety, infection control, post-operative monitoring and emergency care.

Structured Aftercare and Follow-Up

Your care continues throughout recovery with scheduled appointments to assess incision healing, swelling, sensation and the developing lower-face contour. You will also have access to the COSMEX surgical team if questions or concerns arise between reviews.

Book a Jowls Treatment Consultation

Book online via our website to arrange a consultation at COSMEX in Sydney and discuss your jowls, jawline and lower-face concerns. We will assess the location and extent of the sagging before explaining whether facelift surgery, neck lift surgery or a combined procedure may be appropriate.

Jowls and Sagging Jawline FAQs

There is no specific age when jowls begin to appear. Genetics, facial structure, skin quality, sun exposure, smoking, and weight changes can all influence when lower-face sagging becomes noticeable.

Significant or rapid weight loss can reduce facial volume and leave the skin with less underlying support. Whether this makes jowls more visible depends on skin elasticity, age, facial anatomy and the amount of weight lost.

Skincare may support hydration and general skin quality, but it cannot reposition deeper facial tissues that have descended. Facial exercises have not been shown to correct established jowls and may not address the structural changes responsible for a sagging jawline.

Non-surgical treatments may support skin quality or produce limited tightening in selected cases with mild laxity. They do not provide the same degree of tissue repositioning as facelift surgery and are unlikely to correct established jowls.

A facelift addresses descended tissue within the cheeks and lower face, including the tissues that form jowls. A neck lift treats laxity, muscle banding, and loose tissue beneath the jaw, so the appropriate procedure depends on whether the concern is above the jawline, below it, or across both areas.

A facelift and neck lift may be combined when sagging affects both the lower face and neck. Combining the procedures allows the two connected areas to be addressed at the same time, although suitability depends on your anatomy, health and individual risk factors.

Facelift scars may extend around the ear and into the hairline, while a neck lift can also require an incision beneath the chin. The exact position and length depend on the technique used, and all surgical scars are permanent, though their appearance may change as they mature.
