A tissue-preserving approach to breast augmentation

Preservé® by Motiva® is a sub-glandular (on top of the muscle) breast augmentation approach designed to reduce disruption to the existing breast structures compared with conventional dual-plane / under the muscle breast augmentation. This approach is most suitable for patient requesting a more subtle increase to breast volume and less invasive muscle and tissue disruption.

The technique uses specialised instruments to create and prepare the implant space through controlled tissue expansion using a channel separator and a balloon, rather than surgical dissection. The aim is to preserve more of the patient’s existing breast tissue and ligamentous supporting structures during implant placement.

Motiva Preservé® Brochure

Preservé® is performed using Motiva SmoothSilk Ergonomix 2 implants which are softer than the Ergonomics 1 implants and may be considered for selected patients following an individual surgical assessment.

What is Preservé®

Preservé® is a tissue-preservation technique developed for breast augmentation.

Unlike conventional breast augmentation, where the implant pocket may be created through surgical dissection, Preservé® uses purpose-designed instruments to progressively create the space required for the implant above the muscle.

The technique is designed to minimise disruption to the breast’s supportive structures while allowing the existing tissue to be redistributed around the implant.

Preservé® may be considered for:

  • Primary breast augmentation
  • Primary mastopexy augmentation
  • Primary hybrid breast augmentation using implant and Fat grafting

Suitability depends on individual anatomy, existing breast tissue and the type of surgical change being considered.

Suitability depends on individual anatomy, existing breast tissue and the type of surgical change being considered.

How does Preservé® work?

The Preservé® technique uses three specialised Motiva® instruments.

1. Motiva® Channel Separator
The Channel Separator is inserted through a small incision and creates a pathway for the next stage of the procedure.

It is designed to limit disruption to the supportive structures of the breast during preparation of the implant space.

2. Motiva® Inflatable Balloon

The inflatable balloon is then used to gradually and controllably expand the breast tissue.

Rather than creating the implant space primarily through cutting, the tissue is progressively elongated to prepare the pocket and allow the patient’s existing tissue to be redistributed around the implant.

3. Motiva® Insertion Sleeve

The Motiva® Insertion Sleeve is used to assist with placement of the SmoothSilk

Ergonomix2® implant into the prepared breast pocket.
The sleeve allows the implant to be inserted using a no-touch technique, reducing direct handling of the implant during placement.

How is Preservé® different from conventional breast augmentation?

The main difference is the way the implant space is created.
With conventional breast augmentation, the implant pocket may involve more extensive surgical dissection.

With Preservé®, the implant space is progressively created using specialised instruments and controlled tissue expansion.

The technique is intended to reduce disruption to existing breast tissue and supporting structures compared with conventional augmentation.

The manufacturer describes Preservé® as a less-invasive approach compared with conventional breast augmentation.

The most appropriate approach will depend on each patient’s anatomy and surgical requirements.

Tissue preservation
One of the principles behind Preservé® is preservation of the patient’s existing breast anatomy.

The inflatable balloon gradually expands the tissue rather than relying primarily on cutting to create the implant space. This is intended to preserve existing tissue and allow it to redistribute around the implant.

However, Preservé® remains a surgical procedure, and the suitability of a tissue-preserving approach must be assessed individually

Who may be suitable for Preservé®?

Preservé® may be considered for selected patients seeking breast augmentation where the technique is appropriate for their anatomy.

Suitability will depend on factors including:

  • Existing breast tissue
  • Breast shape and position
  • Skin quality
  • Chest-wall anatomy
  • Desired implant size
  • Previous breast surgery
  • Medical history
  • Whether breast lifting is also required

During consultation, your surgeon will discuss whether Preservé® or another breast augmentation technique may be more appropriate.

Risks and Complications of Preservé Breast Augmentation

As with any surgical procedure, breast augmentation carries risks. The decision to proceed should be made after a thorough consultation with a qualified specialist surgeon. The following summarises potential risks; a comprehensive, individualised discussion happens at your consultation, and you can read more about surgical risk.

Potential risks and complications may include:

  • Infection – may require antibiotics or, in some cases, implant removal
  • Bleeding or haematoma – may require further surgery
  • Adverse reaction to anaesthesia
  • Scarring – permanent, with appearance varying between individuals
  • Changes in nipple or breast sensation – may be temporary or permanent
  • Capsular contracture – scar tissue around the implant tightens, causing firmness or discomfort; may require further surgery
  • Implant rupture or deflation – implants are not lifetime devices and may need replacement or removal
  • Implant malposition, rippling or asymmetry – differences in position, shape or feel may occur or persist
  • Breast Implant Illness (BII) – some people report a range of systemic symptoms they associate with their implants; this is discussed at consultation
  • BIA-ALCL – There is no risk of BIA-ALCL with Motiva nano-textured implants. They are considered the same as smooth implants regarding their risk of BIA-ALCL. There has never been a report of cases of BIA-ALCL associated with such implants.
  • Need for revision surgery in some cases implants may displace or

Any surgical or invasive procedure carries risks. This is not a complete list; your surgeon will explain the risks specific to you.

How to Prepare for Preservé Breast Augmentation Surgery

Once your treatment plan is agreed, preparation typically includes:

  • Medical review - health history, medications and supplements, and relevant investigations where required.
  • Medication guidance - you may be asked to stop certain medications or supplements that increase bleeding risk, where clinically appropriate.
  • Nicotine cessation - nicotine can impair healing; you may be asked to stop smoking, vaping and nicotine products before and after surgery for a specified period.
  • Support planning - arrange transport home and help after surgery, especially with general anaesthesia.
  • Fasting instructions - follow the facility's fasting requirements prior to anaesthesia.

Preservé Breast Augmentation Recovery Time and Milestones

Recovery varies between individuals. The milestones below are a guide only; always follow your surgeon's specific post-operative instructions.

Week 1

  • Post-Op Monitoring: You will be carefully monitored as light sedation or general anesthesia wears off[cite: 8]. Mild drowsiness, nausea, and chest tightness are normal early on.
  • Initial Swelling: Your breasts may initially appear larger or over-filled due to retained tumescent infiltration fluid used during the breast tissue preservation technique.
  • Fluid Reabsorption: This localized fluid is typically reabsorbed by your body within the first 1 to 10 days.
  • Support & Mobility: You will wear a surgical support bra to minimize movement. Gentle walking and light, everyday activities are encouraged to promote circulation and ease discomfort.
  • Downtime: Most patients require about one week off work for light duties. Comprehensive written aftercare instructions will guide your recovery at home.

Week 2

  • Improved Comfort: You will feel significantly more mobile, with any residual discomfort managed strictly via your personalized care plan.
  • Exercise Limitations: You can start gentle lower-body exercises (like light walking), but strictly avoid activities that make you hot and sweaty to prevent infection.
  • Wound Care: Your first post-operative dressing change usually happens around 14 days post-surgery.
  • Massage Therapy: You can begin targeted lymphatic drainage massages to help manage fluid retention and support natural healing.

Week 2 - 6

  • Swelling Reduction: Swelling and bruising will progressively decrease, though final breast contours may take several weeks to become fully evident.
  • Activity Resumption: You can safely return to most daily activities and gently increase exercise. Heavy lifting and strenuous chest exercises remain strictly prohibited to protect the implant pocket.
  • Scar Management: Because Preservé™ utilizes smaller incisions than traditional augmentation, targeted scar care is highly effective. Once incisions completely heal, begin daily scar massage with balm, followed by silicone treatments.

Week 6 - 12

  • Fitness Integration: As your body adapts to the SmoothSilk® Ergonomix2® implants, you can safely reintroduce light upper-body exercises into your fitness routine.
  • Lingering Swelling: The vast majority of primary post-operative swelling has resolved, though minor localized firmness may persist.
  • Continued Care: Maintain your daily scar care and massage regimen as instructed by your surgeon to ensure the tissue remains soft and pliable.

3 - 12 Months

  • Implant Settling: Implants will naturally settle into their final anatomical position (the "drop and fluff" phase). As native tissue thickens around the device, the full "tenting effect" takes shape, achieving a natural, proportionate outcome around the implant's rounder edges.
  • Scar Maturation: Scars will continue to fade, flatten, and mature—a biological process taking up to 18 months. Continue treatments until scars are fully mature.
  • Full Clearance: Returning to completely unrestricted, high-impact activities depends on your individual healing progress and official clearance during your final clinical review.

Preservé Breast Augmentation FAQs

  • The procedure is suitable for women desiring natural-looking results, including those seeking primary breast augmentation, mastopexy augmentation, or hybrid augmentation.

  • Ideal candidates should be in a stable physical condition without significant weight fluctuations or immediate plans for pregnancy.

  • Patients must have realistic expectations and understand that final outcomes depend on factors like skin quality, breast density, and tissue elasticity.

  • The procedure utilizes Motiva® Implants in volumes ranging from 95cc to 330cc.

  • Implants are available in Mini, Demi, and Full projections.

  • The unique geometry of the SmoothSilk® Ergonomix2® implants creates a "tenting effect," where native tissue settles around the implant, resulting in a final breast mound volume approximately 25% higher than the implant volume itself.

  • Preservé™ is a less invasive breast surgery utilizing a tissue preservation approach that allows for volume increases of up to 4 cup sizes.

  • Preservé™ is indicated for primary augmentation, tissue preservation mastopexy, and hybrid augmentation, utilizing smaller scars compared to traditional methods.

  • In contrast, Mia Femtech® is a minimally invasive procedure designed for a discreet enhancement of 1 to 2 cup sizes, performed via the axilla without general anesthesia.

  • The redistribution of native breast tissue does not increase pressure on the implant and has not been shown to increase the risk of rippling or wrinkling.

  • The SmoothSilk® Ergonomix2® implants feature TrueMonobloc+® and ProgressiveGel Ultima® technologies, which improve gel-shell integration and reduce the risk of implant visibility and rippling.

  • The initial post-operative breast volume may appear larger due to retained infiltration fluid from the tumescent solution.

  • This fluid is expected to be reabsorbed by the body within 1 to 10 days following the procedure.

  • Additional swelling may persist for several weeks before the final breast contour becomes fully evident.

  • Patients with weak or thinner ligaments, such as those who have experienced excessive weight loss, may require additional stabilization methods and a more conservative implant volume.

  • Sufficient tissue coverage is necessary because Preservé™ implants are placed in the subglandular plane.

  • For patients with minimal tissue coverage, combining Preservé™ with a hybrid surgical approach is an option.

  • The implant base width is determined by identifying the patient's circummammary ligament (CML).

  • Surgeons can manually mark the external and internal borders of the CML or use the Preservé™ Implant Selection set to select a template size.

  • The template's inner diameter should not exceed the CML's internal diameter to avoid violating the breast footprint and to maintain long-term stability.

  • Implant projection should be selected based on the patient's existing breast anatomy to ensure a natural outcome.

  • Surgeons must consider nesting and tenting effects, with Mini and/or Demi projections generally producing the most natural appearance.

  • Surgeons can use implants of two different volumes or projections, though large volume differences can stretch breast tissues disproportionately over time.

  • A recommended alternative for natural results is using same-sized implants combined with lipofilling to correct the asymmetry.

  • For IMF asymmetries smaller than 1cm, the incision is placed in the IMF.

  • For asymmetries larger than 1cm, the incision is placed in the corrected position, taking tissue and IMF elasticity into consideration.

  • Selective pocket positioning determines the trajectory of the Channel Separator and the placement of the Motiva® Inflatable Balloon.

  • If breast volume is evenly distributed, a centrally located tunnel is created to provide overall enhancement.

  • For uneven tissue distribution, the tunnel is directed to position the balloon precisely where projection is needed, such as the supero-medial quadrant for upper pole fullness.

  • According to the tool guidelines, each pump delivers approximately 40 cc of air.

  • For example, a 200cc implant will require 5 full pumps.

  • Acute elongation involves inflating the balloon to the recommended implant volume, then repeatedly deflating and reinflating it.

  • This technique is used for patients with thick, tight skin or dense breast parenchyma to soften and gradually stretch the tissue in a controlled manner.

  • The Preservé™ technique elongates breast tissues radially using the Motiva® Inflatable Balloon without damaging them.

  • The effective balloon diameter decreases after inflation, which allows a smaller balloon to create the framework for a larger implant base width.
